What should you know about vision correction?

Plan to spend 2 hours on the consultation and full eye examination at the centre.

Examination using the newest equipment allows doctors to get the view in good resolution and accurately evaluate the condition of the eye.

Examination before the surgery:

  • Corneal topographic examination.
  • Measurement of intraocular pressure.
  • Examination of eye fundus with dilated pupil.
  • Refraction definition with autokeratore fractometer.
  • Other examinations, if needed.

During the examination, the diameter of the pupil is defined in different lighting conditions. Those measurements are important for creating good vision not only in the daylight, but also at night.

The centre is probably the only institution in Lithuania that additionally inspects, evaluates and plans good vision in the dark before the laser vision correction.

Cornea, pupil, retina, optic nerve are fully examined.

Also, during the visit, the eye condition and general medical condition are fully evaluated and a decision is made as to whether the patient can undergo laser vision correction. The surgery procedure, expectations and post-surgical period are discussed.

In order to ensure the accuracy and reliability of examinations, our eye doctors recommend:

  • Not wearing soft contact lenses for 10 days, or hard contact lenses for 3 weeks before the consultation.
  • Not putting on eye makeup on the day of the consultation.
  • Bring your glasses.
  • Please note! During the consultation, the pupils may be dilated, therefore the vision may be blurred for several hours, and you will not be able to drive and/or work with a computer.

SmartPulse technology affects the eye cornea gently and is minimally invasive, forming the smooth corneal surface. Therefore, good acute vision after the surgery is restored more quickly, e.g. the vision restores within 12 hours after common LASIK surgery, and within 4 hours after SmartPulse surgery.

The graphic shows that when operating with SmartPulse technology, deviations of the laser’s effect on the cornea are smaller, the corneal surface roughness is 3 times less – from one (0.75 µm) to a third (0.25 µm) pulse.

Because of this effect on the cornea, SmartPulse technology is said to be revolutionary.

Having magnified the view by 1000 times with a microscope, a smoother corneal surface can be noticed, which is why the vision after the surgery with SmartPulse is restored three times quicker, the patient feels less pain and discomfort.

At our centre, we use SmartPulse technology for laser vision correction for all methods – LASIK, LASEK, PRK and SmartSurf.

SmartSurf is the least invasive laser vision correction method without an incision, no touching with instruments or chemical substances.

Firstly, the laser evenly and gently removes the epithelium layer without using manual cleaning or alcohol that traumatise the eye and are used in other surgeries. Then, following the data entered by the doctor, the laser smoothly ablates the cornea surface. The laser ray works in the matrix principle in the whole area of the surgical surface. It ablates the corneal surface accurately and evenly both at the centre and on the sides. This guarantees that the vision will be both good in the daylight, and at night. During the surgery, the doctor touches the eye only when preparing it for the surgery, all the work is done by the laser – which ensures accuracy and precision, removing as much of the tissue as is really necessary.

This technology allows the vision correction to be performed on the surface of the cornea, therefore the cornea is not traumatised, the surface remains exclusively smooth. After the correction with SmartSurf, the epithelium regenerates within 36 hours.

Furthermore, surgery with the SmartSurf method means that the patient does not feel suction and pressure, which though not painful, is probably unpleasant. 

SmartSurf does not need any incision, as e.g. LASIK method, therefore there are no related complications, and the eyes do not become dry.

SmartSurf method is suitable for everybody, even those with a thin cornea. Previously such patients, as well as those suffering from diseases of the cornea, and patients with complicated corneal topography could not have had laser vision correction. They would not have had as successful results if the laser vision correction were carried out using another method.

This technology is now being used in the most advanced clinics in Germany, Switzerland, and other European countries.

SmartSurf method is recommended for those who play contact sports, water or air sports, for people whose professions need acute vision (pilots, police officers), or when it is important to safe cornea due to the lifestyle.

SmartSurf is also recommended for patients that need a repeat treatment after radial keratotomy or cornea transplantation. SmartSurf could be  used for treatment of clouded or scarred cornea tissue and keratoconus (thinning and bulging of the cornea, which impairs vision).

Advantages of the laser we use
Fast

The laser’s speed is 1050 laser pulses per second, i.e. f 2 to 3 times more than any other refraction surgery laser system. 1050 tiny flashes per second form the cornea surface, 1,3 seconds for eliminating one myopia dioptre.

Due to the fast speed, the cornea tissue is affected for only a very short time. This protects the tissue against getting dry and speeds up the good vision recovery process. If the laser treatment is too long, the cornea becomes dry – and the treatment accuracy diminishes, the vision correction may be not accurate.

Accurate – softly removes the tissue

Huge speed alone does not ensure the required result. Imagine a picture: a fast yacht moving at high speed turns to a port, and then a small sports boat enters exactly into the narrow place of disembarkation.

The unique automatic laser flow regulating system in the laser ensures a perfect combination of speed and accuracy. At the beginning of the procedure, the laser uses a high-power level, for removal of about 80 per cent of the corrected cornea. How long the rays will be used depends on the strength of the refractive error. It is constantly reviewed by the computer programme during the surgery. The remaining 20 per cent of the corrected cornea is removed by the softer ray. This ensures an especially smooth surface of the cornea, as well as perfect vision. 

Safe and reliable

Imagine that your eye is a buoy in the sea: the sea is calm, and the buoy shifts from left to right, or to front and back. But when there are waves, the buoy shifts to one side. And it also turns around on its anchor. The same happens with an eye during surgery. Therefore, the accurate laser centring and constant redetection of the eye’s position is very important for an accurate and safe procedure.

During the surgery, the eye tracker in the laser registers eye movements in 6 dimensions and at a speed of 1 050 times per second and compensates those movements. This is why the laser ray works directly on the required point. 

Especially thin diameter of the laser ray

Thinner than others – only 0.54 mm – and of a special form, the laser ray forms the cornea accurately and evenly. The small diameter ray means less impact on the tissue.

Integral observing system

The system for measuring corneal thickness (pachymetry) inbuilt in the laser measures the corneal thickness throughout the surgical procedure and ensures that after the laser correction the cornea remains rather stable (thick), since it is like the wall of the eye – it is very important to keep it firm. Similarly, as a diver defines his position in the water with the help of a depth measuring device, during the whole procedure the doctor is informed by the laser computer. This system detects changes in corneal thickness and shows them on the screen on a real time basis. The doctor knows how much has been removed and the remaining thickness of the cornea at any time during the procedure.

A number of other small details not only ensures the effectiveness of the surgery, but also the patient’s comfort during the procedure, e.g. special devices that help to create a stable microclimate in the cornea and thus protects it from getting dry and removes the particles that can obstruct the laser ray and hinder the accuracy of the procedure; a special lamp in the device that enables the doctor to perform needed actions without disturbing the patient.

The actual laser treatment lasts only several seconds. For example, SCHWIND AMARIS removes the required amount of corneal tissue for an 8 dioptre myopia in about 9 seconds. Total time spent on treatment of both eyes is about ten minutes.

The clinical research of SCHWIND AMARIS technology has shown that the patients often see better after the laser treatment than before with glasses or contact lenses. Almost all patients see as well as before wearing contact lenses. The studies also show that eye contrast sensitivity also improves (i.e. the patient sees better in the dark, images and colours are brighter and similar). Even when fully focused, eyes make light unintentional movements that cannot be stopped – as well as when the patient has to focus by looking at a green light during the procedure. Therefore, an accurate and safe procedure depends on accurate centring and constant location detection. The eye tracker in the SCHWIND AMARIS 1050RS laser registers all eye movements and actively compensates them 1 050 times per second.

The ultraviolet light coming from the excimer laser, which is used for removal of part of the cornea, is absorbed by water. This means that the laser affects a dry cornea differently than a wet one. If the treatment time is too long, the cornea starts to get dry and the laser correction is less accurate. The patient’s attention during the laser treatment is also limited, therefore shorter treatment time improves the results.

The eye tracker and eye movement compensation technology integrated in our laser have an important influence on the procedure results even when there is a small (even 1 dioptre) astigmatism – the possibility of deviation and/or error is lower, which means better results.

Medical drops used before the procedure will numb your eye. You might feel a light pressure, but it will not be real pain. A possible sense of a foreign object in the operated eye passes after some time.

There can be a sense of itching in the eyes after the surgery, but usually no greater unpleasant sensation is suffered. In case of a larger discomfort, consult with the doctor, and they will prescribe suitable medicine.

Most patients can return to work on the next day after the surgery, but it is very individual and should be discussed with the doctor during the consultation.

You can do light sports about 3 weeks after the surgery, contact sports about 3 months after the surgery, swimming and sauna procedures – approximately after a month. Recommendations on the regime will be provided by the doctor, or you can ask.

The vision acuity of 100 per cent does not show the maximal capacity, but is only a measure to indicate good vision. Possibilities can considerably exceed 100 per cent.
